Hospice Medical Social Worker - PRN:   Flexible Schedule

Our team is passionate about providing comprehensive care, support, and dignity to those in need. We're currently seeking a PRN Medical Social Worker to join our ever-expanding Hospice team in the Media, PA and surrounding areas. 

What You'll Do

• To function as a resource to patients and families in accessing community services to meet their socioeconomic needs.
• To ensure quality and safe delivery of social services.
• To ensure that social services, reflecting the Agency's mission and standards, are provided in accordance with federal, state, and local standards, guidelines, and regulations.
• To demonstrate support for achieving the mission of the Agency.

About You

• You will have a Masters degree of social work accredited by the Council on Social Work Education
• Prior experience in hospice or palliative care is preferred.
• Strong communication and interpersonal skills.
• Compassion and empathy for patients and their families.
• Ability to work independently and as part of an interdisciplinary team.
• Knowledge of community resources and support services.
• Exceptional organizational and documentation skills.
